If your embedded form isn't appearing, check these things:
1. Is your theme an Online Store 2.0 theme?
The Spoks Embedded Form requires an Online Store 2.0 theme. Older "vintage" themes don't support the kind of inline blocks the form uses.
If you go to Add section → Apps in the Shopify theme editor and can't find the Spoks Embedded Form, your theme is most likely not Online Store 2.0. Check if your theme has an update available that supports Online Store 2.0, and update to use that version or newer.
2. Have you added the Spoks Embedded Form block?
An embedded form only appears where you've added the Spoks Embedded Form block in your Shopify theme. Open the Shopify theme editor and confirm the block is present on the page where you expect the form.

3. Is the correct Form ID pasted into the block?
The block needs the form's Form ID to know which form to display. If the Form ID is missing or incorrect, the form won't show.
Copy the Form ID again from the How to use this form dialog (use the Copy form ID button), and paste it into the block's settings in the theme editor. Save your theme afterward.
4. Is the form's status set to LIVE?
An embedded form only appears when its status is LIVE. A DRAFT form is not displayed. Open the form in the editor and check the Status setting under Form settings.
5. Is the Spoks Onsite Javascript enabled?
Embedded forms need the Spoks Onsite Javascript to be enabled in your Shopify theme. This is a one-time setup. If you've never enabled it, the form won't appear no matter how it's configured.

6. Does the page match the form's URL rules?
If you've set URL rules on the form, the page must satisfy them. With "only show where the URL contains" values set, the page's URL must match one of them. With "don't show where the URL contains" values set, the page must not match any of them.
